Making an Appointment
Please call the Capital Surgeons Group office you would like to visit or the number for the surgeon you would like to see. (Most of our surgeons see patients at multiple offices. Please see the Phone and Fax List below.


What You’ll Need
Please arrive at least 15 minutes prior to your appointment and bring:

• Identification (government-issued, such as a driver’s license)
• Insurance card

Your surgeon will need complete information about your condition, so be sure to also bring of these items that you have:

• Test results
• X-rays
• Mammograms
• Ultrasound
• Any other clinical information from your doctor or the emergency room

You can save time by completing New Patient Forms in advance..


Cancellation
We request 24 hours’ notice if you need to cancel so that we may accommodate another patients.


Your Other Doctors
Capital Surgeons Group will keep your primary care and/or referring doctor apprised of your condition and treatment.
